These Terms of Service (“Terms”) govern your access to and use of elantris.com, related websites, content, intake flows, communications, products, and services made available by Elantris Health, LLC and its affiliates (collectively, “Elantris,” “we,” “us,” or “our”).

By accessing or using the website or services, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the website or services.

Medical Emergency Notice

If you think you may have a medical emergency, call 911, contact your doctor, or go to the nearest emergency department immediately. Do not rely on the website, messaging, intake forms, or asynchronous communications for emergency or urgent medical needs.

Eligibility

The website and services are intended for users located in the United States. Certain services may only be available in states where the relevant providers, pharmacies, coaches, or partners are authorized to operate. You may not use the services if you are prohibited from doing so under applicable law.

You represent that the information you provide is accurate, current, and complete. You agree to promptly update information that changes.

Our Role

Elantris provides technology, administrative, coordination, and support services. Unless expressly stated otherwise, Elantris does not practice medicine, provide medical advice, diagnose conditions, prescribe medication, dispense medication, or operate as a pharmacy.

If you receive clinical services, those services are provided by independent licensed healthcare professionals or affiliated medical practices. Providers are solely responsible for the medical care, advice, diagnosis, treatment decisions, and prescriptions they provide.

Telehealth Services

Telehealth can involve risks, including incomplete information, technical failures, delays, limited physical examination, and potential privacy or security limitations. Telehealth may not be appropriate for every condition or situation.

Before receiving telehealth services, you may be required to review and provide informed consent. You agree to provide complete and accurate information so providers can evaluate whether a service or treatment may be appropriate.

Prescription and Pharmacy Services

Prescription products require evaluation and approval by a licensed healthcare provider. Access to any prescription, medication, treatment plan, or pharmacy fulfillment is not guaranteed.

When a provider issues a prescription, fulfillment may be coordinated through third-party pharmacies or pharmacy networks. Elantris does not guarantee that a medication will be available, appropriate, effective, or filled by any particular pharmacy.

Compounded Medications

Some providers may determine that a compounded medication is appropriate. Compounded medications are prepared by licensed compounding pharmacies for an individual patient. They are not FDA-approved and are not reviewed by FDA for safety, effectiveness, or manufacturing quality in the same way as FDA-approved drugs.

You should review all medication instructions and safety information provided by your provider and pharmacy. Ask your provider or pharmacist any questions before using a medication.

No Guarantee of Results

Individual outcomes vary. We do not guarantee eligibility, provider approval, prescription issuance, treatment availability, weight loss, health outcomes, coaching outcomes, or any specific result.

Website content, testimonials, images, and examples are informational and promotional. They may not reflect typical results.

Account Responsibilities

You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account. Notify us promptly if you believe your account has been compromised.

You agree not to:

• Use the website or services for unlawful, fraudulent, abusive, or harmful purposes.
• Provide false, misleading, or incomplete information.
• Interfere with or disrupt the website, systems, or networks.

If a recurring plan applies, you authorize recurring charges until canceled according to the applicable cancellation terms.

You agree that we and our service providers may contact you by email, phone, text message, in-app message, mail, or other methods you provide, including for account, transactional, service, support, marketing, and compliance purposes where permitted by law.

Message and data rates may apply. You may opt out of promotional communications as described in those messages, but we may still send non-promotional communications related to your account or services.

The website, content, branding, designs, graphics, software, text, images, and other materials are owned by Elantris or its licensors and are protected by intellectual property laws.

Limitation of Liability
